| Part D Program for HIV+ Adolescents, Adult Women, Transwomen & their Children The Ryan White Part D Program is a long running, program of Howard Brown Health Center, which serves HIV+ adult women, transgender women, children, infants and adolescents (ages 13 -24). Participants work closely with a multi-disciplinary team of medical and mental health providers, as well as HIV medical case managers. Case managers in this special populations program have focused areas of expertise, and provide strengths-based case management to all participants. The program currently employs 2 case managers working specifically with HIV+ adolescents, 1 case manager working with HIV+ cis-gender women, and 1case manager working with HIV+ transgender women. Part D at Howard Brown Health Center also provides targeted linkage into care and disease intervention services for all the above populations. Medical Services Our medical providers are specially trained to treat folks living with HIV, and always recognize that HIV treatment can be just one part of what a patient needs. Our providers also provide treatment for a variety of other medical issues, and always work in conjunction with our case managers to provide holistic, comprehensive. Mental Health Services Therapy services are available to participants in the Part D Program and can be specialized to focus on coming out and sexuality issues, dating, violence, family issues, dealing with HIV status, substance use issues and anything else the patient decides is important to them. Case Management Part D Case Managers work with HIV+ participants to coordinate care and provide support around their HIV status, as well as any other issue. Assistance and referrals are available for employment, education, substance use, mental health, medical care, support groups, housing, transportation, alternative therapies and much more. POSITIVE provides HIV+ youth a space to connect with other HIV+ youth (age 24 and under), gain support with issues related to disclosure, dating, medical care, sex and other important topics. This ongoing group meets the 1st and 3rd Thursday of each month at Howard Brown Health Center, 4025 N. Sheridan Road, at 6pm. Dinner is served and transportation available for those in need. ADVOCACY and EMPOWERMENT ADVOCACY and EMPOWERMENT provides HIV+ a youth (age 24 and under) a space to connect and work on current issues of importance in the changing worlds of HIV research, funding and legislation. This activism based group meets the 2nd and 4th Tuesday of each month at Howard Brown Health Center, 4025 N. Sheridan Road, at 6pm. Dinner is served and transportation available for those in need.
