Preload Specialist

REPORTS TO: Chief Technology Officer
FLSA STATUS: Non-exempt, temporary (16 weeks)

General Position Summary:
This position is responsible for extracting health information from paper patient charts and data input into an electronic record. Accuracy and attention to detail required for transferring information to new database system.

Principal Duties and Responsibilities:

Enter data as defined in the Preload Plan from paper charts into GE Centricity electronic health record system.

  • Extract the following predetermined elements:
    • Items to be preloaded
    • Problems - active problems from the Problem List, Summary Page or Adult Health Maintenance Form
    • Medications - from the Problem List, Summary Page or Adult Health Maintenance Form
    • Allergies – front of chart or Medications List
    • Date of last physical exam - from progress notes (most recent note)
    • Height, weight, pulse, BP, Temp. (1 set) – from most recent progress notes
    • In house labs – HCG urine, Urine Dipstick, Stool Guiac, Rapid Strep, Accucheck, Wet Prep
    • Preventive care dates: i.e. pap smears, mammograms, hemoccult , sigmoidoscopy, colonoscopy, - will need to enter results and date for hemoccult results from progress notes or Adult Maintenance Form
    • Immunizations – Td, Hep A, Hep B, Influenza, Pnenumovax, Varicella, PPD, MMR, Hep A & B, Meningitis
    • Psychosocial forms , severity score and date - from Patient Health Questionnaire (PECS)
    • Research Lab data to be entered
    • Pt’s Responsible Providers
    • Referrals without reports
  • BHS Charts:
    • populate the five DSM diagnostic axis's
    • the treatment plan
    • continued stay reviews (only in substance abuse charts)
  • Stamp the chart as PRELOADED and put bright paper in progress notes.
  • Review template information in Centricity for accuracy and completeness.
  • Sign off on preloaded charts and return charts to medical records for re-filing.
  • Preload information according to time measures of no more than 20 minute per chart.
  • Perform health information department quality audits assigned.
  • Demonstrate proficiency with reading and typing health information

Knowledge, Skills and Abilities:

  • Health Sciences or equivalent healthcare work experience of at least 3 years
  • Certified Coding Specialist or eligible
  • Registered Nurse or eligible
  • Medical Office Assistant or eligible
  • Registered Health Information Technician or eligible
  • Knowledge of Medical Terminology
  • Knowledge of Code Sets, ICD-9-CM & CPT
  • Computer Skills-ability to work with computer healthcare software. Prefer electronic health record experience.
  • Ability to examine and identify health record Forms
  • Understand health record documentation: Problem List, Medication List, Progress Notes, Allergies, Immunization records, and order entry systems
  • Ability to perform computer functions with ease and type 40 wpm
  • Ability to communicate in a professional manner, to various members of the health team.
  • Team player who is detail oriented with strong organizational skills
  • Quality of work is imperative
  • HIPAA knowledge
